Your Pharmacy 777 Pharmacist can support you with:
- Medication Management - Our Pharmacists can conduct a Diabetes MedsCheck to review your medications, help you manage your condition and provide health and nutrition advice.
- Blood Glucose Testing – Regularly monitoring your blood glucose levels is important. We can set up your monitor, teach you how to use it correctly, check your technique, and provide guidance on effectively monitoring your glucose levels.
Provide guidance on overall health and wellbeing - Your Pharmacist can provide guidance on maintaining a healthy weight through balanced nutrition, building regular physical activity, and meeting recommended guidelines.
We can also support you with smoking cessation if needed. Managing sleep and stress is important for stable blood sugar levels, and your pharmacist can offer practical strategies to help.
- Referrals – Managing diabetes often involves a full healthcare team, and it’s important to know when to access other allied services. Your Pharmacist can identify these needs and assist with referrals to your GP, Diabetes Educator, Optometrist, Dentist and Podiatrist.
Credentialled Diabetes Educators
Credentialled Diabetes Educators (CDE’s) provide education and advice on all aspects of diabetes self-management care, including where applicable, to educate and evaluate the person with diabetes’ knowledge and skills of injection technique of glucose lowering medicines.
